1把握培训学员特点

本批次我院培训的临床医学高层次人才,年龄35~45岁,拥有医学硕士或博士学位,正高或副高职称,个别人有海外访问学者经历。旨在通过1个月的强化培训,学员能够在门诊、查房、会诊讨论、病历读写等诊疗活动中熟练使用英语与美方教授、医生和同事进行专业的、有效的交流沟通。这批学员英语基础良好,但是也带有成人英语学习者的特点,即记忆减退,少数学员对学习中的困难和挫折不能正确认识,显得意志力不强,注意力不集中[2]。考虑学员以上特点,培训导师首先要取得学员的信任,让他们相信培训有助于自己迅速适应美国医院的工作与生活;其次要多采用启发式、参与式教学法,激发学员的学习主动性。

2设计培训模块

在培训目标的设定上,始终坚持以听说为主、兼顾读写原则,把提高学员在外语环境下的交际能力,满足其出国后工作、学习和生活需要放在首位[3]。共设计4个培训模块,120学时。

2.1公共英语听、说训练

这是培训的初始阶段。此阶段使用日常生活情境中的简单内容,比如见面打招呼、相互问候、自我介绍等,为学员能在日常生活中使用英语交流奠定基础。这不但可以提高学员的自信心和学习英语的兴趣,而且可以唤醒成人学员记忆中的常用英语单词[4]。因为课程实用,词汇简单,并结合相关视频、音频教学,学员容易接受。但此时也要提醒学员,该阶段虽然寓教于乐,但不是我们的终极目标,学员必须为医学英语学习付出艰辛努力。在此阶段,导师讲授用英语进行专业演讲的语言要点,包括开始演讲、提出问题及解答、中断讨论转移话题、结束演讲等。另外,为了提高学员的参与度,让学员先检索未来所在美国城市或医院的资料,再用英语向其他学员讲解。

2.2出国见闻及出国须知

这一阶段培训课时不多,主要是让学员对未来出国情况有感性认识。为了营造良好的语言环境,培训导师应用英语授课。

2.3医学英语听、说训练

经过公共英语培训“热身”后,逐渐进入教学重点,即医学英语培训。我们选取王文秀主编的《医务英语会话》作为教材,同时广泛使用网络视频、音频资料。薛敏等[5]提到,教师应该创造贴近真实的语言交际环境,在课堂教学中,口语活动应该尽可能贴近学员出国后遇到的场景。对于成年人来说,教师更应该注重口语训练,强化学习过程的真实性和可操作性,使语用能力真正融入学员的口语交际能力。以此为理论依据,我们在医学英语课程设置上,提供各类情景对话,如门诊、查房、病历讨论等,并且设立针对各学员专业背景的三级学科。

2.4医学英语精读

众所周知,成人出国英语培训主要是为了提高学员的口语交际能力,而这一能力提高需建立在一定的词汇量基础之上。多做阅读可以帮助学员快速、高效地积累常用词汇和专业词汇[4]。因此,在这一阶段,我们选取倪晓宏主编的《医学英语情景实用指南》作为教材,以门诊病历、住院病历、手术记录等为载体,增加学员的医学英语词汇量,使其熟悉美国医院的医疗文书。

3考核与反馈

经过近1个月的培训,最后进入考核阶段。我们选取英语水平良好的医学专家、英语教授、英语为母语的外教作为考评专家。每位学员就自身专业领域的学科进展进行演讲,考评专家对其英语能力、演讲技巧进行点评,并且就专业领域内容进行提问,考核其用英语进行学术交流能力。此外,我们采用外国留学生作为标准化病人,考核学员用英语问诊以及查房解决诊疗问题能力。结果显示,学员经过培训,基本能满足在英语环境下的学习与工作的需要。学员的反馈是促进教学改革的重要手段,培训结束后,每位学员填写英语强化培训评价表,对课程设置合理性及导师水平进行评价。学员以访问学者身份到美国医院交流期间,我们也追踪学员对培训的评价。两方面的反馈均显示,我们的培训循序渐进,贴近工作和生活实景,效果显著。

针对临床医学高层次人才的出国前英语强化培训,我校国际教育学院教师集思广益,精心设计课程、选取教材、联系专家,开创了新的培训模式,即坚持以听说为主、兼顾读写原则,由公共英语教学逐渐过渡到医学英语教学,理论授课与专业演讲、门诊、查房的场景演练相结合,从而为高级医学人才出国做好充分准备,也为专业教师英语授课提供了宝贵经验。

游是一种户外行为，出国旅行乘车、乘机、坐船的机会多，再加上人生地不熟、语言不通、风俗不同，发生意外事故、不幸生病等风险的可能性之大不言而喻。

旅游保险添保障

正因为出国旅行风险如此巨大，出境前未雨绸缪买份保险以达到保障目的是十分必要的。

出境前在国内保险公司购买保险，是国际上通用的保障方法，许多国家明文规定，游客在出行前必须于所在国购买相关商业保险。2003年12月22日，欧盟理事会就作出了关于旅游医疗保险是签发申请入境签证的附加条件的决定，规定从2004年6月起，凡去欧盟各国旅游和公干的人员，签发申请入境签证的基本条件是必须购买旅游医疗保险，且每人投保金额不少于3万欧元(约合人民币30万元)，此后，俄罗斯、白俄罗斯等国也开始仿效欧盟的决定。

目前，我国保险公司有关境外旅游的保险品种很多。传统的有旅游意外伤害保险，不久前推出的有境外意外伤害及紧急救援保险。值得一提的是后者，它是针对人们出国旅行风险而开办的一项保险业务，除了境外意外伤害保障以外，还具有境外紧急救援多项服务和保障功能。

出境投保多关注

一旦在出国旅行时出险，为了使事后服务能顺利进行，旅客在出境前投保必须认真关注以下7点。

慎重选择各公司

投保时应尽量选择经济实力雄厚、服务网络齐全、社会服务诚信度高，以及有强大实力的救援合作伙伴和服务高效及时的全球救助网络的保险公司。不过，有的国家签证时就已指定入境者必须投保某一家公司所提供的某一项险种。据了解，目前与我国保险公司关于境外意外伤害及紧急救援保险业务合作较好的是国际知名的优普和安盛环球两家国际救援公司。

因地就时挑品种

因为旅游意外伤害保险和紧急救援保险的保险费率都是根据旅行的天数、保障范围额度及旅行目的地国家的消费水平和消费标准而制定的(亚洲国家和北美等国家的医疗消费水平差别较大)，其保险费率也就不同。旅游时间长，风险程度也相对较高，其保险费支出势必也随之增多。因此，出境之前应根据自己的实际需要选择不同保障范围和保障金额的救助保险时段。

妥善保管救援卡

投保出境旅游救援保险之后，每一位旅客将领到一张救援卡，在出国旅途中如需救助服务，可以拨打卡上标明的全球24小时免费中文呼救电话，这样一旦发生就地医疗、护照丢失、语言障碍、法律纠纷等情况就会得到援助服务。为了在旅途中获得方便及时的救援，游客一定要妥善保管好救援卡，最好将救援卡放在身上很容易被找到的地方，以便自己或救护人员及时拨打呼救电话。

明白认知保险单

投保境外旅游险要注意保单生效日期。一般来说，短期险种是可以选择生效时间的，如选择五·一、十·一、春节期间等，而一年期险种通常默认支付保费后次日零时生效。这里要提醒出国旅游者，投保人应于投保时一次缴清全部保险费，因为保险费缴清前，保险合同是不生效的。同时要注意保险单上通常所载的保险金额只是保险公司承担给付保险金责任的最高限额，被保险人在该意外伤害为直接原因而导致身故时，保险人才按保险单所载保险金额给付身故保险金，而如果是因意外伤害致残，保险公司则按伤残程度和相关的比例支付保险金。

谨慎防范假保单

自2004年起，在我国南方如广东等地出现了假保单，这是由犯罪份子自行印制的，销售渠道为通过机场、车站、酒店等正规代售点代售，或者自己销售。假保单险种主要是航空旅客意外伤害保险、旅客意外伤害保险、旅游意外伤害保险和旅游救援等短期险种。一旦买了假保单，不单单是损失保费，更重要的是旅途得不到保障。

为了防止受骗，游客要注意的一是要保证保单销售者用电脑出单，尽量避免手工填写；二是检查保单印制是否粗糙，如果是撕票式保单，就应检查印章是否清晰；三是可以及时拨打保单上标明的免费服务电话进行核实；四是到机场、车站、酒店咨询服务台进行咨询核实。同时，为了防止假保单，有的保险公司在印制意外伤害及紧急救援保险保单时采用了高新技术进行防伪处理，游客购买时应认真识别。

网上投保应仔细

网上投保开始成为一种新方式。由于投保信息是在网上填写，保单生效后，保单受益人以及个人信息就不能再更改，因此填写时必须慎重和仔细。另外，网上投保时投保人往往只保留一个保单号码，一旦出险，要凭着这个号码和相关个人信息向保险公司报案索赔，因此投保人要注意保存保单号码，并在出游前将投保情况告诉亲朋。有条件的投保人还可以将电子保单下载并打印。

一、出生公证、出生证公证

出生、出生证公证所需材料:

① 身份证、户口簿；父母的身份证、户口簿；

② 出生证；

③ 申办出生公证的证明（由申请人档案所在的单位人事部门加意见，加盖公章）；无法提供前述证明的，可提供其他权威机构出具的相应证明。户口簿在同一本上同时直接反映父母子女关系的无需提供本证明；

④ 2寸彩色照片3张（公证书发往美国、韩国、奥地利等国使用的，提供4张，增加1份副本，相应增加1张照片）。办理《出生证》公证无需提交照片）；

⑤ 父母的结婚证（父母离婚的，需提供离婚证、离婚协议书或法院生效的裁判文书）；

⑥ 视情况提供反映出生事实的其他证明资料，如独生子女证，住院分娩资料、由人事档案管理部门出具的人事档案资料（加盖档案保管单位的印章）、计划生育服务证等；

⑦ 如办理《出生证》公证，还应提交医院的证明或原始档案文件（注：如外地出生的申请人提交该材料有困难的，由公证员酌情处理）

二、亲属关系公证

办理亲属关系公证所需资料:

① 申请人的身份证、户口簿；

② 关系人的身份证（港澳台同胞增加提供通行证）、户口簿。关系人为外国人的提供护照；

③ 申办亲属关系公证的证明；

④ 与申办亲属关系有关的证明材料如结婚证、出生证、户口簿；

⑤ 如第④项材料不足以反映亲属关系，则须另提供人事档案简历影印件、公安机关户籍档案证明等并加盖所提供的部门的公章；申请人无法提供前述证明材料的，可提供知晓其亲属关系的其他权威机构出具的证明材料，并出具相关记载文件复印件，加盖公章。如关系人为外国人，其身份的记载与原来的记载已经发生变化，要提供身份变化的证明及关系人在境外对关系认可的声明书，该声明书需经当地国家公证人公证，并经中国驻该国大使馆或领事馆办理认证；

⑥ 公证员综合既有材料后认为应当提交的其他证明材料。

三、委托公证

办理委托公证所需资料:

① 户口簿、身份证及父母的身份证件，申请人没有深圳户口的须带原居住地户口簿、身份证和居住证；

② 出生证或独生子女证；

③ 父母子女关系证明（户口簿能反映父母子女关系的无需提供），非婚生子女提供亲子鉴定书；

④ 父母的结婚证（父母离婚的，需提供离婚证、离婚协议书或法院生效的裁判文书）；

⑤ 委托书/声明书；

⑥ 受托人身份证复印件；
