1.姓名。力求与各种学历证明的姓名相同,如有更改姓名的情况,务必在公证时予以声明,并附上公证书。
2.性别。
3.出生年月日及地点。力求与各类学历证明的出生年月日一致,出生地点写明国别和省份。
4.国籍。
5.婚否。
6.现在工作单位及详细通信地址。
