请假条格式
1、注明标题：
写请假条前首先要在正文上方的居中位置写上标题，一般写请假条时只写“请假条”三个字就可以了。
2、写上称谓：
为了表示尊敬，一般请假条都会写上称谓，比如“XX老师”、“XX经理”等。写称谓时，一般在标题下面第一行顶格写就可以了。
3、开始写正文：
正文中主要写你请假的原因以及要请多长时间，写的时候最好简单明了，只要把大概意思写出来就可以了，不需要具体化。一般请假条的正文大约写两三句就行了。但最后一句话往往会写“请予批准”。
4、写上祝福语：
